She has a dry, warped patch of scales on her head and chips out of the scales on her nose. I give her UVB and supplement. She's at 30% humidity and has consistent access to a humid hide. There's nothing in her tank that she could be hurting herself on. Any ideas?

Looks totally normal to me. Their scales aren't always perfectly smooth, some have little divets in them.

My biggest female Northern has had scales that look like that for at least two years now. I'm pretty sure its normal. Certainly hasn't caused any problems.
